Long and short-term care

To be admitted to our facility we require the following documents:

• chip card
• ID
• ZUS membership card or other document confirming health insurance
• medical records
• doctor’s attestation on health condition – download

Nursing care

The documents, which are necessary to encompass the patient long-term home care:

• referral from your GP or doctor from the hospital department after the completion of the process of treatment in a hospital – Download
• classification of long-term care nurse, according to the formula set out by the NFZ (Barthel scale) – Download
