Nursing services are provided in the patient’s home on weekdays from Monday to Friday from 8:00 to 20:00 and on Saturdays, Sundays and public holidays – in medically justified cases Long-term nursing care is care for the chronically ill and bedridden at home who because of existing health problems require intensive and systematic nursing services provided in collaboration with the primary care physician

Objectives of long-term care
The objective of long-term care nurse is:
- providing patients bedridden nursing services at home
- prepare the patient (and his family) for self-care and to develop skills to cope with disability
- to provide care, in accordance with the nursing process
- health education of the patient and his family members
- help in solving the health problems associated with self-functioning in the home environment
- assistance in obtaining medical equipment and rehabilitation of the patient at home
Conditions for patients taking long-term nursing care
Home care can be provided to patients with chronic illnesses, immobilized due to injury or illness who:
- obtained according to a modified Barthel scale from 0 to 40 points
- are not covered by home hospice care
- are not covered by another care facility providing care services guaranteed under stationary conditions
- are not taken into the care by a team of long-term care services for adults, children and adolescents mechanically ventilated
- not in the acute phase of mental illness

(refunded by the NFZ).

Minimum time allocated for direct care per patient is four visits a week.
